- Uniform Residential Loan Application
You have submitted an on-line loan application. In order to validate the application we need to have your signature attached to the on-line form. Page 3, is nothing more than the signature page that attaches to the on-line application that you have completed.
- Rate Lock Agreement/Credit Card Authorization
Please sign the page and fax it back to us.
- Appraisal Disclosures
This form entitles you to a copy of the real estate appraisal completed on your property when it is completed.
- Information Disclosure Authorization
This form allows us at Mortgage Capital Associates, to verify the information you have indicated on your loan application such as, income, employment and your banking and investment and retirement accounts.
- Equal Credit Opportunity Act Notice
This form establishes the illegality of discrimination to the borrower on the part of the lender. It cites various issues in which a borrower may feel discriminated against and the resources in which they may file a complaint.
- Employment Information Form
This form is very critical in nature. We must have contact telephone numbers, names and pin numbers (in the event your employer has an automated verification process in which to verify your employment and income). Information concerning your employment and income are vital to the qualification and processing of your loan. Please expedite this form as soon as possible.
- Supporting Documents Required For Loan Approval
The key to processing your real estate loan timely is to furnish your documents really f-a-s-t. Most of the documentation can be faxed to us at (888) 477-9035. This will enable the process to move along quickly. Upon receiving your application it is necessary to provide all needed documentation within 48 hours from that time.
